IVF Add-Ons in a Medellín Quote: Ask for Evidence Before Paying
A patient-side framework for PGT-A, embryo-selection tools, immunologic treatments and other extras that can inflate one self-pay cycle.
An add-on is not automatically a scam or a benefit
Fertility clinics may offer tests, laboratory technologies or medications beyond the core IVF pathway. Some may have a specific clinical rationale for a particular patient; others may be offered more broadly despite limited evidence.
The useful response is not to reject every extra. Ask what problem the add-on is supposed to solve in your case and what outcome evidence supports using it.
Current evidence reviews remain cautious
In June 2026, the UK HFEA reiterated that its evidence reviews do not show any reviewed treatment add-on increasing the chance of having a baby for most fertility patients, while acknowledging that an add-on can sometimes have a justified medical use for a specific reason.
That is a strong reason to ask for the clinic’s rationale before spending scarce cycle budget on optional extras.
Price each add-on separately
Ask for the incremental charge, whether it changes medication or travel, and whether it triggers additional freezing, biopsy, storage or laboratory fees. A $500 add-on can become a much larger pathway cost if it changes the treatment sequence.
Then compare that expenditure with the cost of preserving budget for another conventional cycle if needed.
Ask for the outcome that matters
An add-on can improve a laboratory metric without proving that it improves live birth. Ask whether the evidence is about fertilization, embryo selection, implantation, pregnancy, miscarriage or live birth.
If the salesperson cannot distinguish those outcomes, the claim is not useful enough to guide your decision.
Questions worth sending the clinic
- What specific problem is this add-on intended to address for me?
- What evidence supports it for patients like me?
- Which outcome improves: laboratory metric, pregnancy, miscarriage, or live birth?
- What is the complete incremental cost?
- Would you still recommend it if I were not paying out of pocket?
