Do not paste your life story into a lead form
The first contact form only needs enough information to connect you with the right conversation. Sensitive fertility records should move through the clinic’s secure clinical channel once a real provider is involved.
That separation protects privacy and keeps lead capture simple.
Create a one-page fertility timeline
Before the consultation, summarize how long you have been trying to conceive or preserve fertility, prior pregnancies, prior fertility treatment, major diagnoses, surgeries and relevant medications. If there were prior IVF cycles, list retrieval date, eggs retrieved, mature eggs, fertilization method, embryos created, transfers and outcomes if known.
This is much easier for a clinician to review than a folder of screenshots with no chronology.
Then attach the actual reports
Depending on your case, useful records can include ovarian reserve testing, semen analysis, ultrasound reports, hysterosalpingography or uterine-cavity studies, operative reports, genetic testing and prior embryology reports.
Do not order new testing only because an internet checklist mentioned it. Let the reviewing clinic tell you what is current enough and what truly needs to be repeated.
Ask which records must be translated
English-language records may be usable by some Medellín teams, while others may prefer Spanish summaries. Ask before paying for certified translations.
