What the public planning forms collect and how a patient-authorized provider introduction works.
Information you submit
The patient planning form collects basic contact information, country, treatment interest, decision stage, approximate timing, travel readiness, budget/readiness, stated priorities and an optional planning note. The provider application collects professional and business contact information.
Information the public form is not designed to collect
Do not submit medical records, detailed medical history, laboratory reports, genetic information, government identification numbers, payment-card information or other highly sensitive records through the public marketing forms. A treating provider should tell you how to transmit clinical records securely.
Provider introductions
If you explicitly consent on the patient planning form, MedellinIVF.co may share the request and contact information with up to three fertility providers or coordinators that may be able to assist. A general contact message without that consent is not intended to be distributed as a provider lead.
Commercial relationships
Participating providers may compensate MedellinIVF.co for introductions or participation in the referral program. Compensation does not determine medical suitability and should not be treated as a clinical endorsement.
Attribution and technical information
The site may record the page on which an inquiry began, referral source, campaign parameters, browser information and other basic technical data used for analytics, fraud prevention and understanding which pages generate inquiries.
Service providers
Hosting, analytics, form processing, messaging and similar service providers may process information as part of operating the site. Their own terms and privacy practices may apply.
Requests about submitted information
Use the contact page to request access, correction or deletion of information held by this site, subject to applicable recordkeeping or legal obligations.
